Structuring answers

Hey,

I've been told by my teacher and practice examiner that my writing style is clear and fluent, but I'm pretty sure it's my lack of correct structure that loses marks.

Does anyone have perhaps a scaffold or guide as to how to write answers to >5 markers?
At the moment I just go with the flow - answering the question as I see fit.

But when I'm correcting my exams, the answers seem to include stuff like definitions and what seems to be information that isn't being asked for.

Is it actually necessary to be starting every question with a definition?
Is there a simple rule that I can apply to all questions?

We haven't covered this at school so any help is appreciated.

Cheers.
