Transferring after one year of an undergrad - how hard is it?

I'm currently a year 12 set on getting into Physiotherapy at La Trobe which for the past couple of years has been asking for a 96.40, which I realistically believe I can obtain. However, if by any chance I don't make the mark, my plan is to do a one year science undergrad at Melbourne uni then try to transfer over to La Trobe.

Now going by what I've heard, you need to uphold a high grade average throughout the year in order to be considered, but don't know the specifics about transferring. Anyone who has done this/knows of anyone who has/planning to do something along the lines of this, could you please shed some more light on the topic to make it a bit clearer for me and anyone else interested.
