EAS scheme due dates?

Hi,

The key dates for EAS applications for:
On-time applications close on Fri 28 Sep 2012
and Final applications close on Fri 4 Jan 2013
However, does submitting my application on latter date affect anything at all? I mean, does it make difference whether I submit on 28th of Sept or on 4th of Jan?

Just related to EAS, are we allowed to use the same statement of support that we used for SEAS for the EAS application (like just attach it, instead of filling it on the actual sheet? It's just that my doctor is very busy, and I can't get them to fill up the EAS application.
