Title: concession card form

simple question i hope, where do i get one from (clayton campus)?
and then who needs to sign it and how do i do it?
i have no idea, never had one before whooops

What I was told to do was from my.monash, go to the 'Study and Enrolment' tab and under the 'Enrolment' subheading and click the 'Student Services' link. From there, just click the 'Apply for transport concession' link and log in again and fill in the forms and you're done!

I've personally done this and haven't actually received the form yet. Have others had luck with this? Or has anybody had success with just getting the form in person? They didn't let me get it from student services :S

I haven't been sent the form - whenever I try and apply via WES I get the message:

"There has been a problem with the request.
Transport Concessions will be mailed to all eligible students in Mid February prior to the start of semester."

I just got that same message but I thought it was because I'd already applied for one...I'll ask the student services desk tomorrow I guess.

I got mine about a week ago in the mail too!

I just did mine yesterday at the station, took only about 2 mins!

You should've been mailed it.

If anything, I'm sure you can get another copy through that online method (I'm not familiar with this) or from the student services centre.

I talked to some friends today and i didnt realise the forms already had your student ID picture on them. well i didnt get mine taken til monday so i guess that explains why i havent gone mine yet?

I went to the SSC yesterday and got another form printed out, but the photo is black and white and not even positioned within the two boxes (only half of it is lol). I read that the photos had to be coloured and within the set boxes.. but considering it was done by Monash will they still accept it?
