Technically they're not required to ask for permission
under the terms of the Copyright Act the VCAA was permitted to use and adapt written material for exams without seeking prior permission to maintain the security of the exams
Still, poor form. How hard can it be to make up your own friggen piece?

I've just noticed VCAA has now placed an apology on their website: http://www.vcaa.vic.edu.au/index.html
In short, they apologise for not acknowledging her moral rights, and for failing to correctly attribute and editing the article without her permission.
