Title: Form for doings second year subject in first year

So I am such a forgetful gal! I'm wanting to do french that's a second year subject this year because that's where the placement test put me, and I called up monash to ask if that's okay and the nice lady explained that I have to fill out a form to do it. However, I completely forgot the name of the form!! Could someone please tell me what it's called?  :-*
