Tips on writing a conclusion

Can anyone give me some pointers on how to write a conclusion in a prac? I haven't written one in a while and i forgot what's supposed to be included.

Writing a conclusion is meant to be one of the easiest parts of a prac. All you basically need to do is sum up all your findings/resultsand say what the prac has taught you basically or what you have discovered from the prac.

My teacher basically told me to:
1. Answer the aim (what was the calculated result or observation)
2. What Chemistry was involved? (eg. the reactant/s, the reaction type, the product/s)
3. The most significant source of error.
