If you are already a student at UniMelb, they send you a form with your details already filled in for you with your photo. You bring it to the station, hand it in and pay for it so that they can give you a concession card. Alternatively you could ask for a blank form at the station and fill one out (providing a passport sized photo) by yourself.
If the process has changed. You can always go to the customer service area at the station and ask them for a form for a tertiary concession myki. You shouldn't be charged any money for it. It should be free to sign up for a tertiary myki if you're a full time studying student.
