Coherence

Okay, so I know it's been posted before, but I'm so confused.
I went to the VATE lecture, and the lecturer stated for coherence that it's achieved via:
- Deixis
- Inference
- Relations between concepts and lexis
- Varying information

So I understand that, but where does the headings and layout of the text fit into? Are those four points all we can comment on? And if like varying information and deixis is discussed for cohesion, can we use it again for coherence? Arg, so confused.

Headings and layout of the text fits into coherence.  That was on the 2007 exam with the water hose text, which I had a look at the other day.
