From what my teacher has told me, if they give you a picture, it would ALWAYS be accompanied with an actual written prompt. The sole purpose of the picture would be to give the students something that can be used as material for their essays. It's a good idea to incorporate the ideas gained from the picture - not the picture itself - into your essay, but if you're unable to do so, you most likely won't be penalised for it.

This is just what I was told, so if anyone has been told differently, I might be wrong

not necessarily, if you get a picture prompt, then you wont get a written prompt and you have to go with the picture.
You can basically make up your own prompt. But you have to take out the minute things that the picture potrays.

The problem I see with that however, is how does the examiner know what kinda of prompt you are responding to? It seems very broad and would be difficult to mark IMO.

Even if you state it, the picture prompt would be open to interpretation. This could lead to you regurgitating some essay that loosely has 'some' relation to the picture... it just seems a bit :/ If you know what I mean?

Seriously guys, don't worry about something that has such a minute chance of occurring. I mean, technically it's possible that the examination centre you're at might get hit by an asteroid but you wouldn't worry about that would you?

WE COULD GET HIT BY ASTEROIDS? OH NO! *panics*

I don't know what I'll do if I get a visual prompt. I've done a little bit on it in class so I hope that's enough :S
