Cover sheet - urgent

I have an assignment due tomorrow for the French culture component that requires me to hand it in under a completed coversheet - question is, where the hell do I get it? I've looked around but can't find anything for French or other languages.

Menzies building level 4 South Wing - arts office - will have stacks of them. Look for yellow pieces of paper.

Most people do that - it looks better since you have the proper yellow sheet rather than a normal print out. But here you go anyways:
