Submitting evidence'?

Applicants must send in hardcopy evidence of parent/guardian receipt of Family Tax Benefit A. This benefit cannot be collected electronically. Write the relationship of the person to you next to their name on the hardcopy document.

Umm probably a stupid question, but how exactly do I do this? Do I mail it in? What's their address? What do I write on the evidence? Simply "Mother", don't I have to write my VTAC ID??

Somewhere in Myinfo there's a 'documentation coversheet' that you need to put at the front of your file (with a paperclip only), but I'm not sure where to send it either.. :S
