SEAS - What Category?

So I was just distressed from all this exam prep, and decided to jump on to VTAC after a while (finished my regular application a while back) to look up the SEAS categories once again.

I'm applying for Category 1, 2 but have a confusion with 3 and 4.

So towards the end of the 2012 school year, (beginning of Term 4), while playing touch football, a douchebag kid though it would be hilarious to play tackle instead (which I don't mind, but idiotic when not aware). So I'm running with the ball at an angle, he comes and just barges into me, momentum shift, I roll my ankle. Now I didn't just roll it, my ankle went completely inwards, causing a Salter Harris Type 4 fracture (3 breaks right through the ankle, destroying my growth plate. Type 5 is the worst). Had to have surgery, with 5 screws being put into my ankle; had to have 2 different casts, no walking for 3-4 months, only crutches. I was out for pretty much the whole term, had to do my Year 11 Unit 2 exams in the last week of school, and prior to these exams, as typical schools they kicked off Unit 3 studies. So I missed on some content that we did (lame excuse but thats the advantage of SEAS). But then again, I was in a moon-boot half the holidays still recovering right up until Term 1 of this year.

Hence the question, is this a Category 4 or Category 5 classification?

Would be category 5.
Do you have medical documents/letters saying how it has disadvantaged you from that time period?

A bit late to go down that line now. I would have been able to obtain documents, just would've required a phone call to my ortho/surgeon. It's fine anyhow, I'm not too fussed about it.

It's too late to put in the SEAS application now, for but for future reference you don't have to send anything in; there is an online form that you can get your doc to fill out and submit on the spot.

Really? Can you link me it? Could I throw that in as my personal statement by any chance to get some sort of consideration or nah?

It's on the VTAC Website, when you do your Uni Preferences. Click on "Special Consideration (SEAS) Application" in the top menu bar (5th Option). Since its closed, you won't be able to go beyond this point.

It's worth a shot if you put it in your Personal Statement. Click on "Personal Statement" in the top menu bar again (4th Option).
