What should I study and how many hours daily

Only you can answer this question. Just try to cover Year 9 and Year 10 science/maths stuff, and you should be fine. If there's a lot that you struggle with at this level, then probably best to spend a few hours per night or two to understanding that stuff. Depends on what you know or don't know.

Not really important. I wouldn't extend it if I was in your shoes - you're not going to use it much in the future after the exams anyway, if at all.
