My preferred idea is donating to (genuine, effective-altruism-supported hahaha) charities, and giving them the receipt as a gift, and then baking something simple for them.  Honestly, we have everything we need and more than it, why contribute to this affluence when others need it so much more?

Otherwise, joke presents are quite fun! (perhaps more for the giver than the receiver ;))  An eyelash curler for a guy, a fake moustache for a girl, another football club's scarf for an avid fan of one club (I'd never do this ;)), any small gift that plays on some shared piece of knowledge between you or is straight up bizarre.

Judging by the - reaction from Aaron, I'd say by not asking those questions... :D

On the B'day topic...For a 21st, one of my bro's got his mate a bottle of Zacapa XO...

For my (twin) cousins 18ths, my other brother got them matching Swarovski diamond pyramid thingys...
slightly different colours tho... :D

For my Bro's 18, he ended up with Ugg boots (which he 'baptized' that night), two bottles of vodka (1 litre Grey Goose and a bottle of Stolichnaya elit...)
a set of Martini glasses, and one stinking hangover! :D

So yeah....
Alcohol a kinda a go-to for >18, but I think the more thoughtful gifts are the best...
E.G; the Ugg boots last longer than the Vodka... ;)
