Cohorts & rankings & 'state' of school?

Okay something that's been confusing me - people keep talking about being 'ranked 1' in a strong cohort. At first I thought I understood, and now I realise I don't.

What does it mean if you're in a strong school at a certain subject at a certain class/cohort? Does it matter; do you go up, or do they mark you harder, or what? And is it only the class YOU strictly belong to? (For example, we have three English classes and due to timetabling, I'm in a less-proficient class. Still adore them though.)

And what if you're in a school that's average at a subject, but with a strong cohort? And I'm assuming cohorts refer to good grades?

It's confusing me to no extent. Please help before I lose my mind altogether.
