umat registration health care card question

So i've registered for the umat and have to scan my health care card. It states that I have to
'Make a copy of both sides of the card and write your UMAT ID number on it'

Do I physically write it on the card? Like in pen??? + where on the card?
Sorry, I feel like a bit of a newbie T_T

Just scan it on a piece of A4 paper and write your ID number on the paper (not the card)
