Can anyone tell me how to avoid too many CARELESS mistakes please?

Can anyone tell me how to avoid too many CARELESS mistakes please? 
keep a log book of every careless error you make, continually revise this until you kinda have an idea of the type of errors you make, once you try to do your next practice exam make sure during reading time you think about where you have made careless mistakes before and actively make sure you don't make the same careless error again.

Also for your reference book you can contain a list of all your careless errors and once you've completed your exam you should be checking for these careless errors.

No one is really able to stop stupid mistakes but it can be minimised.

I too make careless mistakes (a lot of them) and although I've learnt to recognise some of them, I still fail to spot those mistakes or at least be aware of potential situations in which those mistakes could occur in the heat of the moment. Also, despite being able to correct some silly mistake habits, I find myself replacing them with other new careless mistakes, which are accumulating at a rapid rate. What to do?

forgetting to put in powers in ans to antidiif and dif Q's despite knowing answer (latest trend of mistakes), doing basic arithmetic calculations wrong, not explicitly showing working out for rejecting one answer, using x and  y values for all types of equations even though they may use different pronumerals, getting inverse and derivative equations mixed up... just to name a few (in all seriousness there are more, but I can't think of them from the top of my head).

These silly mistakes are potentially keeping me from attaining a 45 SS or higher. Right now, from doing some past exam 1 papers, I could expect to get are 40 SS. This is the extent to which these mistakes affect me.
