Perhaps one of those deserted island people could figure out the bow drill for fire making. Easier said than done.

That would take intelligence, problem solving skills, perseverance, ability to use trial and error and learn from one's mistakes and the ability to make the best of a hard situation and utilize limited resources - the qualities of a fine doctor imo
